Contribution Statements
Thank you for your faithfulness in giving your tithes and offerings. For the 2018 tax year, your Contribution Statements may be requested January 20, 2019 through March 31st, 2019. Please fill out the form below, and if you have moved in the past 12 months please indicate your previous address.
You must complete all required fields in order for the form to be accepted. This includes Title, (Mr., Ms., Mrs., Dr., etc.), middle initial if applicable, if no middle initial please put n/a (not applicable), also indicate suffix (if applicable) (Jr., Sr, I, II, III etc.) after last name.
